Effective September 2, 2026. These terms are a contract between you and Arizona ESA Letter. By using arizonaesaletter.org or booking a consultation, you agree to them. If you do not agree, please do not use the service.

1. What we provide

We operate a website and a scheduling service that connects Arizona residents with independently licensed Arizona clinicians for telehealth evaluations relating to emotional support animals and psychiatric service dogs. The clinical judgment is theirs, not ours. We do not direct, influence or override their decisions.

2. What we do not provide

  • Legal advice. We are not a law firm and nothing on this site is legal advice.
  • Emergency or crisis care. If you are in crisis, call or text 988, or call 911.
  • Ongoing psychiatric treatment, therapy or medication management.
  • Registration, certification or ID cards. None of these has legal effect in the United States.
  • Any guarantee of approval, or of any outcome with a landlord, airline, employer or university.

3. Eligibility

You must be 18 or older and a resident of Arizona to book on your own behalf. A parent or legal guardian may arrange and attend a consultation for a minor. You agree that the information you provide is truthful and complete. Providing false information voids any letter issued.

4. Telehealth consent

By booking, you consent to receive services by video or telephone. You understand that telehealth has limits, that a clinician may determine an in-person evaluation is necessary, and that technical problems can interrupt a consultation. You may withdraw consent at any time.

5. Fees and payment

Current prices appear on our pricing page. Your card is authorized at booking and captured only after a clinician approves your letter. Fees are one-time. There is no subscription and no auto-renewal. Refund terms are on the refund policy page and form part of these terms.

6. Clinical independence

Clinicians make independent decisions and are not compensated differently based on the outcome. A clinician may decline to issue a letter for any clinically appropriate reason. A decline is not a service failure and is not grounds for a dispute.

7. Acceptable use

You agree not to misrepresent your identity or condition, resell or alter a letter we issue, present an emotional support animal as a service animal in a public place, copy or scrape this site for commercial use, interfere with the site's operation, or use the service for any unlawful purpose. We may suspend or terminate access for violations.

8. Intellectual property

The text, design, logo and layout of this site belong to Arizona ESA Letter and are protected by copyright and trademark law. Your letter is yours to use for its intended purpose. Everything else requires our written permission.

9. Third-party services

Scheduling, video consultation and payment are handled by third-party providers with their own terms and privacy policies. We select vendors carefully and require appropriate agreements, but we are not responsible for their independent acts or omissions.

10. Disclaimer of warranties

The site and service are provided on an "as is" and "as available" basis. To the fullest extent permitted by Arizona law, we disclaim all warranties, express or implied, including merchantability, fitness for a particular purpose and non-infringement. We do not warrant that the site will be uninterrupted or error free.

11. Limitation of liability

To the fullest extent permitted by law, our total liability arising from the service is limited to the amount you paid us for the specific service in question. We are not liable for indirect, incidental, consequential or punitive damages, including a landlord's refusal of an accommodation request. Nothing here limits liability that cannot be limited under applicable law.

12. Indemnity

You agree to indemnify Arizona ESA Letter against claims arising from your misuse of the service, your breach of these terms, or your misrepresentation of an animal's status to a third party.

13. Governing law and disputes

These terms are governed by the laws of the State of Arizona without regard to conflict of law rules. Please contact us first; most disputes are resolved by email within a day or two. If they cannot be, the state and federal courts located in Maricopa County, Arizona have exclusive jurisdiction.

14. Changes

We may update these terms. The effective date at the top reflects the current version, and material changes will be announced on this page. Continued use after a change means acceptance.
